Where is the office and how do I get there?

The office address is at 360 South Monroe Street.   I am in Suite 150, on the first floor to the right if you are facing the elevators.

The office is on the corner of Monroe Street and Cherry Creek Drive North, 4 blocks West of Colorado Boulevard and 1 block South of Alameda.  There are multiple free visitor parking spaces immediately North of the building, as well as free street parking on Monroe Street.  The Cherry Creek bike path runs directly by the office and there are bike racks in the back (park next to mine).  There is easy RTD access via the #1, #3, and #46.

What are your office hours?

Regular hours for patient visits are as follows:

Monday           9:00 am – 4:00 pm

Tuesday          2:30 pm – 7:00 pm

Wednesday     9:00 am – 4:00 pm

Friday             7:30 am – 1:00 pm

Appointments on Thursdays or outside the above hours may be possible on certain days for emergencies or during times of high volume (i.e. flu season)

What insurances do you take?

Insurances I currently accept are listed below as well as in the forms section of the website.

If you have insurance coverage with a health plan I do not accept, you will be responsible for all charges at the time of services.  Please note that some plans will reimburse you a portion of your charges after seeing an out-of-network provider.

It is imperative to present to me and regularly update insurance information at the outset of your care, prior to all visits, at yearly intervals and whenever insurance changes or new cards are issued.   If you have not provided me with updated and accurate insurance information, you may have unexpected financial responsibility for office charges.

I make every effort to verify insurance prior to your appointment.  If I am unable to verify your insurance eligibility, you will be required to pay for your visit at the time of your appointment.  Following the visit, if you provide the correct insurance information to me in a timely manner, I will file a claim on your behalf.  I will refund to you any portion that is determined to not be your responsibility.

It is your responsibility to know the details of your insurance coverage by checking with your employer, insurance agent, or by calling the customer service number on the back of your insurance card.  It is your responsibility to inform me prior to the visit of any limitations in coverage.  The most common instance of this is regarding preventative health visits (annuals / physicals).  Often patients have no coverage for preventative health visits, studies, or tests.  Other times, patients have coverage for preventative health, but are required to pay for regular office visit charges.  Again, it is your responsibility to inform me prior to the visit of any such limitations.

If you have medical insurance, I will submit your insurance forms for payment to your insurance company and will credit such collections to your account.  I will bill you for the remaining portion of your balance, if any, once all insurance claims and payments have been received.  Any co-payments are due and payable at the time of service.  Coding for office visits is done via strict adherence to recognized standards.  Codes for visit charges cannot be changed after they have been submitted.

How do I schedule an appointment?

I use a scheduling process known as open-access scheduling.  This results in more timely care, increased satisfaction for you, and better efficiency for me.  Generally, this means you should always be able to get an appointment within 24 hours for any new problem or follow-up visit.  Preventative annual visits may require several day of advance scheduling.  Most often, follow-up visits will not be scheduled at the time of an appointment, but rather we will both set reminders.  When you are due for your next visit, you will get a reminder (or already have set one yourself); you can then schedule and be seen promptly.

The preferred method to schedule an appointment is through my website.  At the top of the page click on the tab “Schedule an Appointment.”  After seeing the instruction page with up-to-date schedule information, this will bring you to the scheduling page where you pick the type of appointment you need.  From there, simply pick the date and time you prefer.  You will then be asked for some limited information including ALL the issues you wish to discuss at the appointment.  Clicking continue will bring you to a confirmation page where you can review your appointment details and change anything that is incorrect.  Once you click confirm you’re done.  You’ll get a confirmation email as well as a reminder email 24 hour prior to your visit.

Please read carefully the different visit types.  Selecting the appropriate visit type will ensure we have the time needed to address the issues you would like.

You can also request an appointment by calling the office and leaving a message with the reasons for your visit and preferred days and times.

For same day appointments, please use the online scheduler.  It will allow you to schedule an appointment with as little as 1-hour lead time.  If the online schedule is full for the day, please call the office.

Can I be seen without an appointment (Do you accept walk-in appointments?)

I cannot accept walk-in appointments.  It is unfair to those patients that have scheduled visits.  It also takes time I have set aside for responding to patient’s phone calls and electronic messages.

How do I contact the office?

There are several ways to contact the office outside of office visits.  The preferred route is by  electronic messaging.  You can also contact the office via telephone or by fax.  Please see descriptions below.

For any life-threatening or acute medical issue, you should call 911 or go the nearest emergency room; do not call the office or use online messaging for these issues.

How do I contact the office by electronic messaging?

Electronic messages can be sent via traditional email.  Electronic messages are appropriate for questions regarding non-urgent medical advice, for new issues or follow-up from prior visits (including most test and study results), scheduling issues that cannot be handled by the online scheduler, and billing/insurance issues.

The confidentiality of e-mail exchanges cannot be guaranteed.  If your e-mail address is through your employer, your employer may own all e-mails sent to that address.  If your e-mail address is a family address, other family members may see your messages.  If you use an internet service provider, there is a small risk that messages may be intercepted by others (“hackers”).

The following subjects are never appropriate for electronic communication (e-mail or secure messaging):

  • Any urgent medical problem or emergency

  • Mental health issues

  • Drug and alcohol problems

  • HIV and other sexually transmitted diseases

What happens when I call on the telephone?

There is no receptionist at my office.  When you call the office, either I will personally answer, or you will be directed to the messaging system.  The messaging system is more than just an answering machine; it will find me if you call with an urgent issue, convey pertinent office information, and have a general voice mailbox for non-urgent needs.

If I do not answer the phone and the issue requires immediate attention, please press 1 and the system will attempt to find me on my cell phone or other locations.  Often, I will simply answer; otherwise, you can leave a message.  I will be immediately informed of all urgent messages and in fact get a written transcription of your message (so speak clearly and slowly).  I will then respond as soon as possible.  Please ensure only urgent matters are present when you choose this option and respect the time I have seeing other patients in the office or with my family.  For non-urgent issues, please leave message in the general voice mailbox.  I will check messages regularly throughout the day.  Please remember that I will check and respond to electronic messages prior to voice messages.

What happens where you are out of the office?

There will be a variety of times I am not available for office visits.  Often, I will be in town (or even in the office) researching issues for patients, handling non-medical office issues, or educating myself.  I will certainly take time off to spend with my family, pursue personal interests, or take vacation.

I will update my phone system and website daily for any changes to my routine schedule.  In the future, I will also likely implement social networking services (Facebook, Twitter, etc.) to inform you of schedule changes.  You will be informed of vacation times in advance so that you may ensure you issues are met prior to those times (specifically refill and referral requests).  My vacation time is likely to fall within the Denver Public Schools break schedule.  The office will generally be closed for 1-2 days around the following holidays.

  • New Years
  • Memorial Day
  • 4th of July
  • Labor Day
  • Jewish High Holidays (generally in September or October)
  • Thanksgiving
  • Christmas

If I am unable to see you in the office, and you feel physician evaluation is necessary, I recommend you go to urgent care.  MedExpress urgent care center is 1 block from my office (http://www.medexpress.com/clinics/colorado/cherrycreek/cherrycreek.shtml).  There are also physicians with whom I have partnered that may have availability for same day visits for acute issues.  A list of those offices and Denver area urgent care centers can be found in the forms section of the website.  For severe medical complaints, I recommend going to your nearest emergency department.

For most routine time off, and even much vacation time, I will continue to be accessible both by telephone and secure online messaging.  Response times may be slightly slower; however my website and phone system will be updated with expected response times.

My interests often take me and my family to the mountains during time off where I may be in and out of cell phone range.  If you do not get a response in a time frame you were expecting, please just try again.  If it is an issue that truly cannot wait for me to respond, it is likely best handled by an urgent care or emergency department visit anyways.

For extended vacations or times when I know I will not be accessible by phone or messaging, my phone system will direct you to another physician that is covering my calls.

How do I get refills?

If you need a refill from me for a prescription medication that you take on a daily or continuous basis, please contact the pharmacy first.  They will either fill the prescription or contact my office to request an authorization.  If the pharmacy cannot handle the request, you may request a refill by contacting me.  If I have not seen you within the past year, I will fill one month worth of medication and require an office visit prior to further refills.  If you need a refill for a prescription medication that you do not take regularly, I will probably need to see you first.

Please allow at least 24 hours for all prescription refill requests during the week and at least 72 hours on weekends and holidays.

For mail order pharmacies, I request that you use the online refill request.  Please indicate you require a 3-month supply for mail order.  If your mail order pharmacy accepts electronic prescriptions, I will send it myself.  If not, I will send the prescription to you and you can send to your mail order pharmacy.  I prefer this method as opposed to faxing or calling in prescriptions.

What is the procedure at office visits?

Since I have no office staff, all your interactions at the office will be directly with me.

When you arrive for your appointment, please have a seat and I will be out shortly to greet you.  If by chance you did not complete any necessary paperwork or online forms prior to your visit, you will be given a chance to do so when you arrive.  I will collect all updated demographic and insurance information and collect necessary co-pays.  Prior to you leaving, I will collect any office visit charges that are required at the time of service.  I will print out any prescriptions, referrals and lab or study orders that are pertinent.

There are many steps you can take to ensure the office runs smoothly and that your time there is spent productively.

  • Scheduling the appropriate visit type and listing all your concerns you wish to discuss at the time of scheduling
  • Completing all necessary paperwork prior to your visit.  All forms are available for download in the forms section of the website.  Some may be completed online.
  • Bringing your ensuring card to all visits and ensuring you inform me whenever insurance or demographic information changes
  • Arriving promptly for your visits

Can I get samples at the office?

I will not have any samples at the office.  I feel strongly about limiting the influence of pharmaceutical companies in my practice.  I keep up with the medical literature and am aware of new medication treatments on the market.  Sometimes, these treatments are breakthrough advances that significantly improve my ability to treat you.  However, more often than not, new medications are simply old medications repackaged or reformulated without any significant improvements over our older agents.

If there is a newer medication that you’d like to discuss with me, please schedule an appointment or send me an online message.  We can review whether the medication is appropriate for you.  Should we decide to try it, I can write a 5-10 day prescription for a trial period.

Can I get vaccines or medication shots at the office?

I will not be providing vaccinations at the office.  I am, however, a strong advocate for patients receiving recommended vaccinations.  I will do everything possible to ensure the lack of availability at the office does not in any way decrease the likelihood you receive appropriate vaccines.  Nearly all major pharmacies now offer routine vaccinations such as influenza, shingles, pneumonia, tetanus-diphtheria-pertussis, measles-mumps-rubella, meningitis, HPV and hepatitis.  Additionally, I can write prescriptions for vaccines that you can pick up at the pharmacy and bring to your appointments;  I can then administer in the office.  If there is a vaccine you know you need, please either contact your pharmacy directly, or contact me and I will help set that up.  At preventative exams, we will review all necessary vaccines and provide a list of recommended shots you should receive at the pharmacy.

I will still be providing B-12 injections for those that have documented B-12 deficiency and prefer injection therapy.  I will have a limited supply of other injectable medication.

Where will I get my blood drawn?

Blood draws should be done at convenient Quest Diagnostics or Labcorp locations.  I prefer patients use Quest Diagnostics if possible.  At this time, the only insurance plan of which I am aware that requires patients to use Labcorp is Anthem / Blue Cross / Blue Shield.  The closest Quest Diagnostics center is 2 ½ blocks away from my office on South Madison Street (see map here).  I will not be doing blood draws at the office.

I am on warfarin/coumadin.  How do I get my levels checked?

Currently, patients on warfarin/coumadin will need to go to a lab to have their INR (blood thinner level) checked.  Please let me know your preferred laboratory location and I can set up a reoccurring order so you do not need to bring an order with you when you go.  I will get results within 48 hours and contact you regarding any dosages changes.

Unfortunately, I am no longer able to check blood thinner levels in the office.  I realize this was a convenient method.  However, since I have very few patients on blood thinners, there was no way I could justify the time and cost necessary to provide this option.

There are some other potential options that I am pursuing, especially for those that do not do well with routine blood draws.  First, some pharmacies may be able to do point-of-care testing (finger poke) and send results to me.  Secondly, it is possible to do home monitoring of your INR.  In that situation, you have the device at home and check your own level with a simple finger poke and a drop of blood.  You then either contact me with your result, or follow an action plan I have previously given you.  These options are not available yet, but I am currently looking into whether these are realistic options.

How do referrals to specialist physicians or other providers work?

Often there are times when we may feel specialist evaluation is necessary.  Additionally, many patients have chronic conditions which are co-managed by a specialist.

I am fortunate to work with a plethora of excellent specialists, both medical and surgical based at Rose Medical Center.  Additionally, there are a variety of physicians throughout the Denver Metro area with whom I regularly share patients.  Most often, the decision to refer you to a specialist will occur at an office visit; I will give you one or more referral choices at that time, if you do not have a preferred physician.  Patients may also request a specialist referral for an issue I have not reviewed.  Sometimes, I may recommend you are first seen for an appointment to discuss the issue.  Other times that will not be necessary and we can get you names of providers if you do not have a specific preference.  If you think you need a referral, please either schedule an appointment or send me a secure message with the reasons for your request and any providers you wish to see.

I also work with a wealth of non-physician providers in a variety of fields.  Ancillary providers to which I commonly refer are physical therapists, psychologists, chiropractors, acupuncturists, and massage therapists.  Again, this is usually discussed at office visits but if there are issues for which you feel one of these providers are needed, please let me know.

I generally send a written referral to all other providers, both to provide your medical background and reason for consultation, as well as to ensure the specialist knows I am your primary physician.  Insurance sometimes requires this written referral and sometimes do not.  Additionally, some insurance may require preauthorization prior to paying for visits with other providers.  It is your responsibility to know whether the specialist takes your insurance as well as what paperwork or preauthorization is required.  If there is something you need to facilitate this, please let me know.  I would recommend you confirm anything required is complete prior to being seen as certain authorizations may not be possible after the fact.

Please make sure all other providers know that I am your primary physician and stress with them the importance of sending me complete records after every visit.  Should they recommend labs, studies, or procedures, please ensure those records are also sent to me.

How do I get radiology or other diagnostic tests?

Frequently an issue will require radiology studies or other diagnostic tests.  Nearly always this will be decided at office visits, though there are instances where studies may be ordered without an office visit.

There are multiple locations for radiology procedures, both in the immediate vicinity of my office, as well as throughout the Denver Metro area.  The locations closest to my office are Rose Outpatient Imaging on the Rose Medical Center campus, ImageOne in Lowry and Cherry Creek Imaging in Cherry Creek.  If you have a preference, please let me know.  A printable list of radiology locations is below, as well as in my office forms section.

Other diagnostic tests can be done at various locations throughout the city.  Preferred locations will depend on the type of test and will be discussed at the time I order the study.

Often your insurance may require a preauthorization for a test I order.  When I send the order to the office performing the study, I send all necessary information so that office can take care of any necessary preauthorization.  If they cannot get that accomplished, I will do it.  However, it is your responsibility to know whether a preauthorization is needed.  Please do not do any test until you are positive that the necessary preauthorization, if required, has been achieved.  Even with insurance and with appropriate preauthorization, you may have a financial responsibility for a test.  Please make sure you are aware of all your costs prior to doing a test.

What happens if I need to be admitted to the hospital?

For any emergency or life threatening issue, I recommend you either call 911 or go the nearest emergency department.  If admission is needed, you will be admitted at that facility by an inpatient (hospitalist) physician.

If the issue is stable enough that you have a choice on where to go, I recommend you go to Rose Medical Center.  I have close relationships with the physician on staff there and can monitor your course closely.  At Rose Medical Center the medical service is managed by physicians from National Jewish Health.  They will direct all aspects of your hospital course.  Specialist physicians will be consulted as necessary.  I do not formally admit patients at Rose or other hospitals.  However, I will play a role in monitoring your hospital course and assisting with admission and discharge planning to ensure your transition from home to hospital and back to home is a smooth process.

Should you require rehabilitation care after your hospital stay, it will be arranged prior to your discharge.  I do not directly manage patients in rehabilitation facilities but again will assist with discharge planning and ensure a smooth transition back to home.

What do I do if I want to transfer care to another physician or get my records?

Prior to ending your relationship with my practice, I recommend you first find another physician.  This way, there are no gaps in your care and you always have a primary physician.  Once you have found another physician, you should request that records be sent to that physician.  You can fill out a release at the new physician’s office or use my form.  A release of information to transfer your records can be found here or on in my patient forms section.

If you request your records be transferred to another physician, there will be no charge.  There will be a charge if you have records released to yourself.  This will be for all releases sent after 10/7/09.  The fee schedule is listed below.  These fees are subtantially less than the fee schedule dictated by the Colorado State Medical Board (see section 5.2.3.4 of state document here).  If your chart costs would be cheaper per the State Board's Fee Schedule, you will be refunded the difference.  If you are requesting records be sent to yourself, please include appropriate payment with your release.  Please note that records cannot be picked up directly at the office.  All records must be mailed.

Can I see both you and Dr. David Amli?
