How's Your Health?

In order for me to gain further insight about my patients and my practice, I will be starting to use the How's Your Health online health screening tool.  This assessment will allow me to help you better understand your health risks and conditions and your hundles to effective prevention and treatment,  I am in the process of finalizing my registration and will soon be able to start collecting data.  You will see links to the How's Your Health screening tool on appointment reminders as well as being on the scheduling page.  Once I have my registration code, I will update this page so everyone can being using this excellent tool to understanding and improving health outcomes.
